Since January 2026, all 18-year-old men receive this form. Here's what it contains, how to answer, and why it matters.

What is this questionnaire?

The "Questionnaire for Assessing Military Service Suitability" is sent to all men aged 18 and older (born 2008 and after). Women may participate voluntarily. For men, filling it out is mandatory – failure to respond may result in a fine.

Pre-existing conditions, physical and mental limitations

Current occupation, training, field of study

How should you answer?

Some important principles:

  • Be honest – but strategic. The questionnaire is for assessment, not evaluation.
  • Check "conscientious objection" early if it matches your beliefs. This shortens the process.
  • Health information is relevant – but consult a doctor before strategizing here.
  • You don't need to provide detailed motivation. A simple "No" on grounds of conscience is sufficient.
  • Keep a copy of your completed questionnaire.
  • Appeals & legal remedies are powerful. If you receive a fine: file an objection. Every single case requires manual review – and that clogs the system. More in the FAQ.
